Tennis Elbow (Lateral Epicondylitis): Causes, Treatment and Recovery

Tennis elbow, medically known as lateral epicondylitis, is a common cause of persistent outer elbow pain that affects a meaningful proportion of adults at some point in their lives. Despite its name, it has little to do with tennis for most sufferers. It is far more commonly seen in people whose work or hobbies involve repetitive gripping, twisting or wrist extension, including manual trades, computer-heavy desk work, racquet sports and weight training. The condition can be frustratingly persistent, often lingering for months, and understanding what is actually happening in the tendon helps explain why the most effective treatments look quite different from simply "resting until it goes away".

What is actually happening in the tendon

The extensor carpi radialis brevis tendon attaches to a bony bump on the outer side of the elbow called the lateral epicondyle. Repeated gripping and wrist extension place repetitive tensile load on this tendon. In earlier stages, this can trigger a genuinely inflammatory response, tendinitis in the strict sense. However, in most men who present with symptoms lasting more than a few weeks, biopsy and imaging studies show that the tendon has instead undergone a degenerative process, disorganised collagen fibres, increased ground substance and abnormal blood vessel ingrowth, with little active inflammation present. This is properly termed tendinosis rather than tendinitis, and the distinction matters because purely anti-inflammatory treatments tend to underperform in tendinosis, while treatments that stimulate tendon remodelling and progressive loading tend to work better.

Understanding this shift in thinking, from an inflamed tendon that simply needs rest, to a degenerated tendon that needs a structured reloading programme, is central to modern management of tennis elbow.

Recognising the symptoms

Pain is typically felt over the outer elbow and can radiate down the forearm towards the wrist. It is usually worsened by gripping activities such as shaking hands, lifting a kettle, turning a doorknob or a key, or extending the wrist against resistance. Many men first notice it when their grip feels weaker or when a previously trivial task, like carrying shopping bags, becomes uncomfortable. Pain at complete rest is uncommon in the early stages; it is the provocation with specific movements that is the hallmark. As the condition becomes more chronic, some men notice more constant background discomfort and a subtle but real reduction in grip strength that affects daily function.

What causes it, and why it becomes chronic

The underlying driver is a mismatch between the load repeatedly placed on the tendon and the tendon's current capacity to tolerate that load. This can happen acutely, following a sudden increase in activity such as a new manual task, an intense session of racquet sport after a long break, or a change in work equipment, but more often it builds gradually through sustained repetitive strain, such as prolonged computer mouse use, tool handling in trades like carpentry or plumbing, or repetitive lifting.

A key reason tennis elbow becomes chronic is that most people cannot simply stop using their hand and forearm the way they might rest a sprained ankle. Work and daily activities keep re-loading the tendon before it has had a chance to properly remodel, which is one reason the condition can smoulder for many months if not actively managed with a structured plan rather than passive rest alone.

Load management: the foundation of recovery

The first and most important step is not complete rest, but relative load management, identifying and modifying the specific activities that provoke pain while keeping the tendon moving within a tolerable range. This might mean adjusting grip technique, using ergonomic tools, taking more frequent breaks during repetitive tasks, or temporarily reducing training intensity in racquet sports. Complete immobilisation is generally discouraged because tendons respond poorly to prolonged inactivity, becoming weaker and less able to tolerate load once activity resumes.

Eccentric loading exercises

The single most evidence-supported active treatment for lateral epicondylitis is a structured eccentric loading programme. Eccentric exercise involves lengthening the muscle-tendon unit under load, for example slowly lowering a light dumbbell held with the wrist extended, rather than lifting it. This type of controlled, progressive loading appears to stimulate collagen realignment and tendon remodelling more effectively than rest or stretching alone.

A typical programme starts with light resistance and a small number of controlled repetitions performed daily, gradually progressing the resistance and repetitions over several weeks as symptoms allow, under the guidance of a physiotherapist. Consistency matters more than intensity; sporadic, aggressive exercise sessions tend to aggravate symptoms, while steady, progressive loading over weeks to months tends to produce durable improvement. Most structured eccentric loading programmes run for six to twelve weeks before a clear improvement is felt, and full recovery of pain-free function can take several months.

Bracing and supportive measures

Counterforce braces, worn just below the elbow, work by redistributing load away from the site of tendon irritation during gripping activities. Many men find these helpful for symptom control during work or sport, though bracing is a symptomatic adjunct rather than a cure, and should be used alongside, not instead of, a loading programme. Wrist splints that limit wrist extension can also help during flare-ups by reducing tendon strain, particularly at night or during specific provoking tasks.

Injections versus ESWT: comparing procedural options

When a genuine trial of load management and structured exercise (generally at least six to eight weeks) has not produced adequate improvement, procedural options become worth considering.

Corticosteroid injections can provide rapid short-term pain relief by reducing local inflammation, which can be useful for breaking a painful flare and allowing exercise to progress. However, evidence suggests that the benefit is often short-lived, and some studies have raised concern that repeated corticosteroid injections may weaken tendon tissue and lead to worse outcomes at the six to twelve month mark compared with exercise-based treatment alone. For this reason, corticosteroid injections are generally used sparingly and are not considered a long-term solution.

Extracorporeal shock wave therapy (ESWT) takes a different approach, delivering focused acoustic energy to the affected tendon to stimulate local blood flow, cellular activity and collagen remodelling, essentially encouraging the tendon's own repair processes rather than simply masking pain. ESWT is typically delivered over a course of several sessions, spaced roughly a week apart, and is generally well tolerated with minimal downtime, allowing most men to resume normal activity immediately afterwards. It is particularly useful for men with more chronic, degenerative tendinosis who have not responded adequately to exercise alone, and is often considered before more invasive options.

Realistic recovery timelines

Setting expectations correctly is important, because tennis elbow does not typically resolve overnight. Mild, recently developed cases may improve within four to six weeks with prompt load management and exercise. More established cases, particularly those present for several months before treatment begins, often take three to six months of consistent, structured rehabilitation to substantially improve, and a small proportion of men have symptoms that persist beyond a year despite good adherence to treatment. Surgery is rarely required and is generally reserved for the minority of cases that remain significantly symptomatic after a thorough trial of conservative and procedural treatment lasting six to twelve months.

Men who return too quickly to the activities that originally provoked the condition, without modifying technique or gradually rebuilding tendon capacity, are at meaningfully higher risk of recurrence. Building tolerance gradually, rather than chasing a quick fix, gives the best chance of a lasting recovery.

Differential diagnosis: ruling out look-alike conditions

Not every case of outer elbow pain is lateral epicondylitis, and a careful clinical assessment helps avoid months of treatment aimed at the wrong structure.

Radial tunnel syndrome results from compression of the radial nerve as it passes through the muscles of the forearm, close to the same area affected by tennis elbow. It can occur alongside tennis elbow or mimic it closely, but tends to produce a more diffuse, aching pain located slightly further down the forearm rather than directly over the bony epicondyle, and is often worsened by resisted forearm rotation rather than wrist extension alone. Tenderness on examination is typically found over the muscle belly rather than the bony prominence itself, and grip strength testing with the elbow extended versus bent can help distinguish the two conditions.

Cervical referral from the neck is an important and frequently overlooked cause of elbow symptoms. Irritation or compression of a nerve root in the neck, from degenerative changes or a disc bulge, can refer pain down the arm and into the outer elbow and forearm, sometimes accompanied by neck stiffness, pins and needles reaching the hand, or symptoms that do not consistently track with gripping activities. A doctor will typically screen neck movement and check for reproducible symptoms with neck positioning if this pattern is suspected, since treating the elbow alone will not resolve pain originating from the cervical spine.

Elbow osteoarthritis is a less common but genuine differential, particularly in men over 50 or those with a history of previous elbow injury or heavy manual work. Osteoarthritis tends to produce pain that is more diffuse across the joint rather than localised to the outer bony prominence, is often accompanied by stiffness, a reduced range of elbow movement, and sometimes an audible or palpable grinding sensation with movement, none of which are typical of straightforward tendinosis. X-rays can confirm joint space narrowing or osteophyte formation where this is suspected.

Because these conditions can overlap or coexist with tennis elbow, and because their management differs meaningfully, ruling them out through a careful history, targeted examination and, where indicated, imaging or nerve studies, ensures the treatment plan actually matches the underlying problem.

Planning a return to sport and work

Returning too early, or too aggressively, to the activities that triggered tennis elbow in the first place is one of the most common reasons for relapse, and a structured return plan reduces this risk considerably.

For men returning to racquet sports or gym-based training, a graded approach generally works better than an all-or-nothing return. This typically means resuming light, technique-focused practice before returning to full-intensity play, checking that grip strength and pain-free range of movement have substantially recovered, and considering an equipment review, grip size, string tension or racquet weight can all influence load on the extensor tendons and may need adjusting even after symptoms settle. Warming up thoroughly and avoiding a sudden jump in session frequency or intensity in the first few weeks back are simple measures that meaningfully lower re-injury risk.

For men in manual or repetitive-strain occupations, such as carpentry, plumbing, machine operation or extensive keyboard and mouse use, a phased return that gradually reintroduces full task intensity, rather than resuming a full workload immediately, allows the tendon to continue adapting under real-world conditions. Simple ergonomic changes, adjusting tool grip size, alternating tasks to avoid prolonged repetitive strain, or reviewing workstation set-up for desk-based roles, can reduce the likelihood of the original mismatch between load and tendon capacity recurring. Where symptoms are significant, a short period of modified duties, agreed with an employer and supported by a doctor's letter if needed, can be a reasonable bridge back to full function.

In both settings, using persistent grip strength or continued pain with specific movements as a guide, rather than a fixed calendar date, gives a more reliable indication of true readiness than simply counting weeks since the pain first eased.

Do I need to have played tennis to get tennis elbow?

No. Most cases occur in people whose work or daily activities involve repetitive gripping or wrist movements, including manual trades, desk work involving repetitive computer use, and weight training, rather than racquet sports specifically.
