Anonymous STI testing - what it covers

Anonymity and confidentiality are not the same thing, and the difference matters legally. Here is exactly where the line sits.

Anonymous: chlamydia, gonorrhoea, syphilis

Your sample is processed against a code rather than your name, and you collect the result with that code. Nothing about the test is attached to your identity. Let our staff know in advance so the sample is labelled correctly from the outset - it cannot be made anonymous after the fact.

Confidential, not anonymous: HIV

In Singapore, anonymous HIV testing may only be carried out at MOH-designated anonymous test sites. HIV testing at Hisential is fully confidential - results go to you and your treating doctor and are never shared with employers, insurers or family without your consent, except where required by regulations or law, but it is not anonymous. How we keep rapid testing accurate

Speed is only worth having if the result holds up. This is the methodology behind each rapid option we offer.

In-house PCR for chlamydia and gonorrhoea

Nucleic acid amplification testing (NAAT) is the reference standard for chlamydia and gonorrhoea in both WHO and US CDC guidance - it amplifies and detects the organism's own genetic material rather than inferring infection from an immune response. Running it in-house rather than couriering samples to an external laboratory is what makes a 4-hour turnaround possible at all. Each run carries positive and negative controls, and any invalid run is repeated rather than reported.

Rapid syphilis with a confirmatory pathway

Rapid syphilis testing uses a treponemal antibody immunoassay on a finger-prick sample, read in 10-15 minutes. Treponemal antibodies persist for life, so a reactive rapid test alone cannot distinguish current from previously treated infection. Every reactive result is therefore followed by RPR (a non-treponemal titre that reflects disease activity) and TPHA confirmation before any diagnosis or treatment decision is made.

3rd versus 4th generation HIV testing

A 3rd-generation rapid test detects HIV antibodies. A 4th-generation test detects antibodies plus the p24 antigen, which appears in blood before the antibody response is measurable - shortening the window period from roughly four to six weeks down to two to four. Where the exposure is very recent and the clinical stakes are high, HIV RNA/DNA PCR detects the virus earlier still. Reactive rapid results are always confirmed on venous blood.

Which results stand on their own, and which do not

The rapid HIV and rapid syphilis test kits are screening tests. A negative result inside the window period still needs a repeat at the appropriate interval, and a reactive result triggers confirmation the same visit rather than a diagnosis. The chlamydia and gonorrhoea PCR sits in a different category: it is the confirmatory method itself, so a positive PCR result stands on its own and treatment can be started on it. Turnaround and window periods

Testing too early inside the window period can produce a false-negative. Speed and timing are separate questions - this table covers both.

When a full panel is the better choice

Rapid testing answers a narrow, urgent question. If you want the broader picture - hepatitis A, B and C, herpes, mycoplasma, ureaplasma, trichomonas - a comprehensive panel is both better value and a more complete answer.   1. Is anonymous STD testing really anonymous?

    For chlamydia, gonorrhoea and syphilis, yes - your sample is processed against a code rather than your name, and you collect the result using that code. Tell our staff before the consultation so the sample is labelled correctly from the start. Anonymous testing is not available for HIV: under Singapore rules, anonymous HIV testing may only be carried out at MOH-designated anonymous test sites. HIV testing with us is fully confidential, except where disclosure is required by regulations or law, just not anonymous.

  2. How fast are the results?

    Rapid syphilis and rapid HIV are read in 10-15 minutes during your visit, with no time cut-off. Chlamydia and gonorrhoea are run on our in-house PCR with a 4-hour or same-day option - provided the sample is taken before 3:30pm. After 3:30pm, the PCR result is reported the next morning.

  3. Why does the 3:30pm cut-off exist?

    PCR is a batched laboratory process - extraction, amplification and analysis take several hours end to end. A sample received before 3:30pm can complete a full run and be reported the same day. Anything later finishes overnight and is released first thing the next morning.

  4. How accurate is rapid testing?

    The PCR (nucleic acid amplification) we run in-house for chlamydia and gonorrhoea is the method WHO and the US CDC both recommend as the reference standard - it detects the organism's genetic material directly rather than inferring it. Rapid syphilis uses a rapid test kit that detects treponemal antibodies; any reactive result is confirmed with RPR and TPHA before a diagnosis is made. The rapid HIV test kit is a screening test in the same way, and a reactive result is confirmed on venous blood.

  5. What's the difference between the 3rd and 4th generation HIV test?

    The 3rd generation test detects HIV antibodies only. The 4th generation test detects antibodies plus the p24 antigen, which appears earlier - so it shortens the window period and can pick up infection roughly two to four weeks after exposure rather than waiting for the antibody response alone.

  6. What if a rapid result is positive?

    It depends which test it is. The rapid HIV and rapid syphilis test kits are screening tests, so a reactive result is not a diagnosis: it triggers confirmation the same visit, on venous blood for HIV and with RPR and TPHA for syphilis, and your doctor talks you through what the preliminary result does and does not mean before you leave. The chlamydia and gonorrhoea PCR is different. PCR is the confirmatory method in itself, so a positive PCR result is a confirmed result and treatment can be started on it, usually at the same visit. 

  7. Can I be treated anonymously too?

    For chlamydia, gonorrhoea and syphilis, treatment can be provided during the same visit. Some steps - prescriptions and any notifiable-disease obligations - do require identification, and your doctor will explain exactly where that line sits before anything is done.

  8. What if my exposure was in the last 72 hours?

    Rapid testing this soon after exposure will not yet be reliable - you are still inside the window period. If the exposure was high-risk and within 72 hours, HIV PEP should be considered immediately; see our HIV PEP page and contact the clinic the same day.

  9. Should I choose rapid testing or a full panel?

    Rapid testing answers a specific, urgent question fast. If you want a broad picture - hepatitis, herpes, mycoplasma, ureaplasma, trichomonas - a comprehensive panel from our STD testing menu is the better value and the more complete answer.

  10. Can I use MediSave or insurance at Hisential?

    Fees are private and payable in full at the visit. MediSave, CHAS and Healthier SG subsidies do not apply. Some insurance plans cover consultations and investigations on a reimbursement basis: you pay the clinic and claim back from your insurer afterwards. The concierge provides the documentation you need and helps with the claim paperwork. Instalment payment is available: ask the concierge about the options before your visit.
