Quick Answer

Longevity screening at Hisential Singapore measures three things that change over time and respond to treatment: your epigenetic biological age, your gut microbiome, and your physical function, including VO2 max and resting metabolic rate. The programme runs in four cumulative tiers, The Janus, The Apollo, The Helios and The Orion, each building on the one below. Every tier ends the same way, with an SMC-registered doctor translating the numbers into a plan you can follow.

The three pillars of longevity data

No single test defines how you are ageing. We measure three layers because each one shows something the others cannot, and all three can be moved.

How you are ageing now. DNA methylation testing estimates your biological age, which is distinct from your chronological age and sensitive to sleep, training, nutrition and stress. Repeat it to see what is working.

Learn more

Your microbial ecosystem. Gut microbiome analysis reports diversity, the balance of beneficial and opportunistic organisms, and what your microbes are functionally doing for digestion, immunity and inflammation.

Learn more

How your body performs today. VO2 max on a cycle ergometer and resting metabolic rate by indirect calorimetry give measured fitness, training zones and true daily energy needs instead of wearable estimates.

  • Peak oxygen uptake and ventilatory thresholds
  • Heart-rate training zones from your own data
  • Measured resting energy requirement

Why healthspan, not just lifespan

Most people focus on lifespan, meaning how long they will live. The more useful question is healthspan, meaning how many of those years are spent in good health. The gap between the two is the years lived in poor health, often concentrated in the final decade. Longevity screening exists to measure the things that influence that gap while they can still be changed.

Illustrative example, not a patient result.

The goal is not to flag every possible problem. It is to give you clarity. With real measurements in hand, you and your doctor can make more personalised decisions about training, nutrition, screening and treatment.

Four cumulative tiers

Each tier includes everything in the tier below it and adds a further layer of measurement. Every tier covers pre-test consultation, sample collection at our Orchard Road clinic, laboratory analysis, clinician-led interpretation, a written report and an action-plan session. Your personal health concierge confirms the full inclusions and fees for your chosen tier before anything is booked.

Optional add-on

Add a Health Screening to Any Tier

The longevity tiers measure how you are ageing and how your body performs. A health screening adds the clinical bloodwork and imaging layer, which fine-tunes how your biological age is interpreted and looks for disease at an early, treatable stage. Either package can be added to any tier and reviewed in the same session.

Self-check

Is longevity screening right for you?

A confidential, 30-second self-check. If two or more apply, a calm consultation is a reasonable next step. Your responses stay on this device only.

If you ticked only one, that is still a valid starting point. Come in and we will discuss which tier suits you.

This self-check is informational only. A full clinical consultation is conducted before any test is recommended.

Why people choose Hisential

Personal Health Concierge

One dedicated contact who coordinates consultation, sample collection, laboratory tracking, results review and your follow-through plan, end to end.

SMC-Registered Doctors

Data is only useful when an SMC-registered doctor turns it into advice you can act on. Our medical team reviews every report before you see it.

Measured, Not Estimated

VO2 max on a cycle ergometer and resting metabolic rate by indirect calorimetry, rather than figures inferred from a watch or a formula.

Built to Be Repeated

Every layer we measure can change. Tiers are designed around a baseline plus repeat readings so you can see whether your plan is working.

Understand the tests

The pathway in depth

Every tier begins with a doctor consultation. We review your history, current symptoms, training, family history and what you want out of the programme, then confirm which tier fits.

Your personal health concierge is introduced at this point and stays with you through collection, reporting and follow-through.

There is no obligation to proceed on the day. Many patients take the recommendation away and book later.

Quick answers

Q:

How long does it take to get my results back?

It depends on the laboratory handling each sample. Your concierge confirms the timeline at collection and tracks every sample until your review is booked. VO2 max and RMR results are available on the day.

Q:

Is any of this covered by insurance in Singapore?

Coverage for preventive and predictive testing is limited. We provide an itemised invoice for personal claims, though most patients pay out of pocket.

Q:

Can I share my results with my regular doctor?

Yes. Your written report is yours, and we can coordinate directly with another clinician on your behalf.

Q:

Can I start at one tier and move up later?

Yes. The tiers are cumulative, so patients often begin with The Janus and add the functional or performance layers at their next review.

Q:

Do you offer whole genome sequencing?

No. The Singapore programme focuses on measures that change over time and respond to treatment: epigenetics, the gut microbiome and physical function.

FAQ

Frequently asked questions

Clear answers, written by our clinical team. Tap any question for its direct permalink, or reach out to your Personal Concierge for anything else.

  1. Why screen for longevity if I feel fine?

Most biological change happens quietly for years before symptoms appear. Longevity screening measures what is happening now, so modifiable findings such as an accelerated biological age, a dysbiotic gut or poor cardiorespiratory fitness can be addressed while there is still room to act.

  2. What does longevity screening at Hisential actually measure?

Three layers. Epigenetics gives your biological age from DNA methylation. Gut microbiome analysis reads the microbial ecosystem shaping digestion, immunity and inflammation. Physical function testing, including VO2 max and resting metabolic rate on a cycle ergometer, measures how your body performs today.

  3. How are the four tiers structured?

The Janus, Apollo, Helios and Orion tiers are cumulative. Each one includes everything in the tier below it and adds a further layer of measurement, so you can start at the level that fits your goals and step up later.

  4. Do you offer whole genome sequencing?

No. Genome sequencing is not part of the Singapore longevity programme. Our screening focuses on measures that change over time and respond to treatment: epigenetics, the gut microbiome and physical function.

  5. Is biological age testing reliable?

The epigenetic clocks we use are peer reviewed and validated. A single reading is a snapshot, so the number is most useful as something you track. We recommend a baseline plus a repeat reading 6 to 12 months later to separate real change from measurement variation.

  6. Can the results predict when I will get sick?

No, and we are honest about this. These are measurements, not verdicts. Their value is in what they let you and your doctor do next, not in what they forecast.

  7. How often should each measure be repeated?

Epigenetic age every 6 to 12 months while you are actively making changes. Gut microbiome every 3 to 6 months while changing diet or treating dysbiosis, otherwise annually. VO2 max and resting metabolic rate every 3 to 6 months during a structured training block.

  8. Is the gut microbiome kit easy to use at home?

Yes. The kit contains everything you need with written instructions, collection takes a few minutes, and the sample is shelf stable for several days at room temperature before drop off or courier collection.

  9. How long do results take?

It depends on the laboratory handling each sample. Your personal health concierge confirms the reporting timeline when your samples are collected and tracks every sample until your action-plan session is booked.

  10. Will I need additional tests afterwards?

Sometimes. If a result warrants confirmation, your doctor will recommend the next step. Anything outside your tier is quoted separately before it is arranged.

  11. Can I use MediSave or insurance at Hisential?

Fees are private and payable in full at the visit. MediSave, CHAS and Healthier SG subsidies do not apply. Some insurance plans cover consultations and investigations on a reimbursement basis: you pay the clinic and claim back from your insurer afterwards. The concierge provides the documentation you need and helps with the claim paperwork. Instalment payment is available: ask the concierge about the options before your visit.

  12. Can I share my results with my own doctor?

Yes. The written report is yours, and we can coordinate directly with another clinician on your behalf if you prefer.

Glossary

Biological age
An estimate of how old your body is biologically, as opposed to how many years since you were born. Measured most reliably with epigenetic clocks.
Chronological age
Years since your date of birth. A fixed number that does not reflect how well your body is functioning.
DNA methylation
Small chemical tags attached to specific positions on your DNA. The pattern changes with age, lifestyle and disease, and is what epigenetic clocks measure.
Dysbiosis
An imbalance in the gut microbial community, typically reduced diversity, overgrowth of certain species, or under-representation of beneficial ones.
Epigenome
The collection of chemical tags and structural features sitting on top of your DNA that control which genes are switched on or off in each cell.
Epigenetic clock
A statistical model that estimates biological age from patterns of DNA methylation.
Healthspan
The portion of your life lived in good health. The goal of longevity work is to extend healthspan, not simply lifespan.
Indirect calorimetry
The method used to measure resting metabolic rate, calculated from the oxygen you consume and the carbon dioxide you produce at rest.
Microbiome
The collective community of microorganisms living in and on your body, most densely in the gut.
Resting metabolic rate (RMR)
The energy your body uses at complete rest, which forms the base of your daily calorie requirement.
VO2 max
The maximum rate at which your body can take in and use oxygen during exercise. Measured here on a cycle ergometer with breath-by-breath gas analysis.
Ventilatory threshold
The exercise intensity at which breathing rises disproportionately to workload, used to set training zones.
