Daily PrEP is the most studied, most flexible, and most commonly used PrEP regimen.

Two daily formulations are available in Singapore: TDF/FTC and TAF/FTC. TDF/FTC is the most studied regimen and has evidence across exposure routes, including receptive vaginal sex. TAF/FTC is a newer tenofovir prodrug with generally more favourable kidney and bone markers, but trial evidence is limited to men who have sex with men and transgender women; it is also associated with modest weight and lipid increases. Both are dispensed as originator tablets, not generics. The choice between them is made at consultation based on kidney function, hepatitis B status, exposure type and cost.

The schedule: one tablet of TDF/FTC or TAF/FTC every day at approximately the same time. Most patients take it with breakfast or with another daily medication to support adherence. Take with food to reduce mild gastrointestinal effects.

Time to protection: adequate tissue concentrations build over several days - approximately 7 days of daily dosing for full protection against rectal HIV exposure, and approximately 21 days for full protection against vaginal HIV exposure. Before these intervals, use condoms or avoid potential exposure.

Stopping daily PrEP: continue daily dosing for at least 7 days after the last potential exposure (28 days after the last unprotected vaginal exposure). This ensures protection extends through the last exposure window.

Adherence support: pill organisers, phone alarms, linking PrEP to an existing daily habit (breakfast, brushing teeth, bedtime), tracking apps, and honesty with your clinician about missed doses - adherence patterns guide ongoing prescription.

Best for: anyone whose situations are not predictably planned, anyone who finds a daily routine easier than thinking in advance, women (where on-demand dosing is less established), regular partner activity, and anyone newly starting PrEP - daily provides the most clinical certainty in the early phase.

PrEP prevents HIV. DoxyPEP - a single 200 mg dose of doxycycline taken within 24 - 72 hours after condomless sex - reduces chlamydia and syphilis by 70 - 90% (and gonorrhoea partially). Many patients on PrEP also take DoxyPEP as part of comprehensive sexual-health care.

DoxyPEP is only recommended for MSM (men who have sex with men) and transgender women. The dPEP-Kenya trial showed no benefit in cisgender women, and there is no efficacy data in heterosexual men - so Hisential does not prescribe DoxyPEP outside that indication.

  1. What does PrEP stand for?

    Pre-Exposure Prophylaxis. It is medication taken before potential HIV exposure to prevent infection if exposure occurs. PEP is the related Post-Exposure Prophylaxis - taken after exposure to reduce transmission risk. PrEP and PEP are complementary tools.

  2. How effective is PrEP at preventing HIV?

    Approximately 99% effective when taken as prescribed. Effectiveness drops with missed doses. Daily PrEP and on-demand 2-1-1 dosing (when used correctly) provide comparable real-world protection for men who have sex with men.

  3. Do I need a prescription for PrEP?

    Yes. PrEP requires prescription after a baseline HIV-negative test and safety workup. It is not over-the-counter. In Singapore, PrEP is dispensed through private clinics like Hisential.

  4. Can I use MediSave or insurance at Hisential?

    Fees are private and payable in full at the visit. MediSave, CHAS and Healthier SG subsidies do not apply. Some insurance plans cover consultations and investigations on a reimbursement basis: you pay the clinic and claim back from your insurer afterwards.   5. Will PrEP show up on a drug test?

    PrEP is not a controlled substance and would not be detected by standard recreational drug screening. PrEP could be detected by very specific HIV-medication assays, but these are not used in routine workplace or insurance testing.

  6. Can I stop PrEP if I want to?

    Yes - under clinical guidance. For daily PrEP, continue daily dosing for at least 7 days after the last potential exposure (28 days for vaginal exposure) to ensure protection extends through the last exposure window. Hepatitis B carriers require coordinated planning before stopping due to risk of hepatitis B flare.

  7. Will PrEP affect my fertility?

    Current evidence does not show PrEP affects fertility, sperm parameters, or pregnancy outcomes. PrEP is sometimes used specifically during conception planning where one partner is living with HIV.

  8. Is PrEP safe in pregnancy?

    Yes - current evidence supports PrEP safety in pregnancy and breastfeeding for women at ongoing risk of HIV exposure. The clinical decision is individualised.

  9. Can I drink alcohol on PrEP?

    Yes, in moderation. Alcohol does not directly interact with PrEP medications. However, heavy alcohol use can affect adherence (missed doses) and decision-making around sexual activity.

  10. Will PrEP work for receptive anal sex if I just took the first dose?

    For on-demand dosing, take 2 tablets at least 2 hours before sex - the 2-hour minimum is essential. For daily dosing, full protection for rectal exposure builds after approximately 7 days of consistent daily use.

  11. Should my partner also be on PrEP?

    Possibly - depending on your partner's HIV status and own risk profile. If your partner is living with HIV and not yet on effective treatment (or has not achieved undetectable viral load), PrEP for you is particularly important. If your partner is on effective HIV treatment with undetectable viral load (U=U), sexual transmission risk is already eliminated, but PrEP may still add benefit for additional protection layers.

  12. What happens if I have an exposure while not on PrEP?

    HIV PEP is the appropriate intervention - a 28-day antiviral course started within 72 hours of exposure. After PEP, transition to PrEP can be discussed for ongoing prevention.

  13. How long can I stay on PrEP?

    Indefinitely, as long as the indication continues and quarterly monitoring is maintained. Many patients are on PrEP for years. Some take breaks during periods of lower exposure and resume later - this is fine but requires the same baseline workup before restarting.

  14. Is treatment confidential?

  15. Is it safe to buy PrEP from foreign online sources?
