Shoulder Slimming with Botulinum Toxin

Shoulder slimming uses botulinum toxin in the upper trapezius where a bulky, high shoulder line is muscular. It is assessed and injected by an SMC-registered doctor.

Abstract still life: a long sculpted stone form with a gently sloping curve, standing in for shoulder contour treatment.

Quick answer

Botulinum toxin placed in the upper trapezius softens the slope of the shoulder line over weeks where the bulk is muscular. It is not a treatment for posture, and if you train or lift heavily the effect on strength is discussed first.

What it is

The upper trapezius runs from the neck to the shoulder. Where it is bulky, the neck can look short and the shoulder line high, and reducing its activity softens that slope over weeks.

This is a functional muscle. If you lift, carry or train, reducing its activity has consequences, and that conversation happens before treatment rather than after.

Assessment and dosing

Trapezius assessment
Your doctor examines the muscle at rest and under load to judge bulk and symmetry.
Dosing and load
Dose is set against your training and daily load, and reviewed at follow-up.
Neck and posture
Neck pain and posture have other causes. This treatment is not offered as a posture correction.

What it treats

  • A bulky upper trapezius that shortens the appearance of the neck
  • Asymmetric shoulder bulk in selected cases
  • Muscular shoulder line where the concern is contour, not pain

The procedure, step by step

Assessment first: your doctor examines the upper trapezius at rest and under load, to judge bulk, symmetry and how much work the muscle does for you.

Injection points are marked along the muscle belly between the neck and the tip of the shoulder.

Injections are placed into the trapezius. The dose is larger than a facial dose because the muscle is larger; session length is set by your doctor at consultation.

Results timeline

The shoulder line softens and the neck looks longer as bulk reduces. Strength under load can soften with it, which is the trade-off to weigh before treatment. Results vary between individuals and cannot be guaranteed.

How quickly botulinum toxin acts, and why a review is booked before any further dose, are covered on the anti-wrinkle injections page.

Downtime and aftercare

Pinpoint marks and occasional bruising. Aching across the shoulder for a day or two is common, and lifting or carrying can feel weaker for a period. Report any weakness that interferes with daily activity.

The general aftercare for any toxin treatment, including the twenty-four hour restrictions, is set out on the anti-wrinkle injections page.

Course and maintenance

The effect wears off, so this is repeated treatment. Interval and dose are set at review against how much load the muscle actually carries for you.

Product and provenance

Botulinum toxin is a prescription-only medicine and is described here in generic terms rather than by brand. The product used and its batch number are recorded in your notes at the time of treatment.

Why patients choose Hisential for this

  • Every injection is performed by an SMC-registered doctor, not delegated.
  • If you lift, carry or train, the effect on strength is discussed before treatment rather than discovered after it.

FAQ

Frequently asked questions

Clear answers, written by our clinical team. Tap any question for its direct permalink, or reach out to your Personal Concierge for anything else.

  1. Is this permanent?

No. Trapezius bulk returns as the effect wears off, particularly if you keep training the muscle, so this is repeated treatment rather than a one-off change.

  2. When will I notice a difference?

Not on the day. The shoulder line softens gradually over the weeks after treatment, and the neck looks longer as it does.

  3. Is the muscle the actual cause?

That is what assessment establishes. A high shoulder line can be skeletal or postural, and neither responds to toxin, so your doctor examines the trapezius at rest and under load before offering treatment.

  4. Will it affect my training?

It can. Reduced trapezius activity may be noticeable in lifting and carrying, which is discussed and dosed for before treatment.

  5. Who performs the injections?

Every injection at Hisential is performed by an SMC-registered doctor.
