Calf Slimming with Botulinum Toxin

Calf slimming uses botulinum toxin in the calf muscle where the bulk is muscular rather than fat. It is assessed and injected by an SMC-registered doctor.

Abstract still life: a slender sculpted stone column with a subtle swell tapering to a fine line, standing in for calf contour treatment.

Quick answer

Botulinum toxin placed in the calf softens muscular bulk over weeks. It is a functional muscle used in walking, running and standing, so the effect on activity is discussed and dosed for before treatment.

What it is

The visible bulk of the calf is mostly the gastrocnemius. Where that muscle is genuinely prominent, reducing its activity softens the contour over weeks.

The calf does real work every day. Dosing is conservative and staged, and if you run or train seriously your doctor will set expectations about that before anything is injected.

Assessment and dosing

Calf assessment
Your doctor examines the calf standing and on tiptoe to judge which part is bulky and whether it is muscular at all.
Dosing and gait
Dosing is conservative and staged because the muscle is load-bearing, and it is reviewed at follow-up.
Training load
Runners and lifters are dosed and reviewed differently, and in some cases advised against treatment.

What it treats

  • Prominent calf contour that is muscular in origin
  • Asymmetric calf bulk in selected cases
  • Muscular calves where the concern is contour rather than function

The procedure, step by step

Assessment first: your doctor examines the calf standing and on tiptoe, to establish which head of the gastrocnemius is prominent and whether the bulk is muscular at all.

Injection points are marked across the medial and lateral heads of the muscle.

Injections are placed conservatively and in stages, because the calf carries you all day. Session length is set by your doctor at consultation.

Results timeline

The contour softens gradually as the gastrocnemius reduces, and because the muscle is load-bearing the change is deliberately partial. Results vary between individuals and cannot be guaranteed.

How quickly botulinum toxin acts, and why a review is booked before any further dose, are covered on the anti-wrinkle injections page.

Downtime and aftercare

Pinpoint marks and occasional bruising. Calf aching or a feeling of heaviness for a few days is common, and stairs, running and tiptoe work can feel weaker for a period.

The general aftercare for any toxin treatment, including the twenty-four hour restrictions, is set out on the anti-wrinkle injections page.

Course and maintenance

Dosing is staged, so the plan is judged over more than one session rather than settled at the first. The effect wears off, and the interval is set by your doctor at review.

What determines the cost

What moves the cost of calf slimming is the dose the calf muscles need, which is large by comparison with facial dosing, and whether one or both legs are treated.

FAQ

Frequently asked questions

Clear answers, written by our clinical team. Tap any question for its direct permalink, or reach out to your Personal Concierge for anything else.

  1. Is this permanent?

No. Calf bulk returns as the effect wears off, and it returns faster if you run or train the muscle hard, so treatment is repeated at an interval set with your doctor.

  2. When will I notice a difference?

Not on the day. Calf contour softens slowly over several weeks, and because dosing is staged the fuller picture comes after the second session.

  3. Is the muscle the actual cause?

That is what assessment establishes. A heavy-looking calf can be fat or bone rather than gastrocnemius, and your doctor checks on tiptoe before offering treatment, because neither of those will respond.

  4. Will it affect walking or running?

It can, particularly with running, jumping and prolonged standing. Dosing is conservative for that reason and the trade-off is discussed before treatment.

  5. Who performs the injections?

Every injection at Hisential is performed by an SMC-registered doctor.
